Intermediate - Non-pitting surface rust on food-contact equipment. Observed on dry dish racks.

Basic - Accumulation of debris on exterior of warewashing machine. Observed exterior soiled at time of inspection.

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed water damage to several tiles near triple sink. Repeat Violation

Basic - Floor soiled/has accumulation of debris. Observed in employee cafeteria where soda syrup dispensers are.

Basic - Reach-in cooler shelves with rust that has pitted the surface. Observed in multiple coolers in kitchen.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed soiled shelf where dry dishes are held.

Intermediate - Non-pitting surface rust on food-contact equipment. Observed on dry food racks and dry dishes racks.

Basic - Floor tiles missing and/or in disrepair. Observed in employee cafeteria.

High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed sweet plantains (129 F - Hot Holding) at warmer precipitation area. As per operator for less than 30 minutes. Operator moved the product to the bottom part of the warmer to reheat the product. Corrective Action Taken

Intermediate - Spray bottle containing toxic substance not labeled. Observed spray bottle with peroxide not labeled at preparation table cafeteria. Operator labeled the spray bottle. Corrected On-Site

Intermediate - Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed water at 76°F after three minutes running water at hand wash sink women bathroom. As per operator the hot water is shut off.

Basic - Cutting board has cut marks and is no longer cleanable at kitchen area.

Basic - Water leaking from pipe and/or faucet/handle. Observed water leaking at hand wash sink.
