Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. No paper towels at hand wash sink located at ware-washing area or preparation area. Warning

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. At front equipment needing some exterior cleaning.

Basic - Wet mop not stored in a manner to allow the mop to dry.

High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed a hose not having a vacuum breaker at mop sink.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed vents soiled just above menu board.

Basic - Employee with no hair restraint while engaging in food preparation. Observed employee engaged in food preparation not wearing a hair net. Employee put on a hair restraint. Corrected On-Site

Basic - Exterior door has a gap at the threshold that opens to the outside. Observed an air gap bottom of back door.

Basic - Stored food not covered. Observed sausage patties stored in walk in cooler not covered.

Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed wet towel in use not stored in sanitizer solution front counter next to wand washing sink. Manager placed it in sanitizer solution.

Intermediate - Records/documents for required employee training do not contain all of the required information. Observed employee date of birth missing from certificates.

Basic - Missing drain plug at dumpster.

Basic - Ten (10) foot-candles of light at least 30 inches from floor not provided in walk-in cooler.
