Manager stated that three new employees started last week, manager not able to provide documentation that employee responsibilities were explained to them , employees did not have food handler certifications. Sent employee health agreements to Manager who printed them out and had employees sign them. Corrected On-Site

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Nozzles on tea dispensers near drive through window soiled with calcium like substance. Old coffee residue build up on coffee maker Nozzle near drive through window.

Intermediate - No soap provided at handwash sink. No handwashing soap/ dispenser in men's restroom.

Intermediate - Spray bottle containing toxic substance not labeled. Observed unlabeled pump spray bottle with purple chemical in it, manager was not aware of what chemical was in it.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Multiple ceiling tiles in kitchen area soiled with dust.

Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee drink on food prep table next to soda dispenser behind front counter. Manager relocated drink. Corrected On-Site

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. In dining area at soda dispenser, upper section of ice chute has mold-like substance.

Basic - Equipment and utensils not properly air-dried - wet nesting. At dish area, clean metal pans stacked before air drying.
