High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb: - mop sink has a splitter on the faucet with two hoses connected to it. One hose is going up to the chemical dispenser and there is a long black hose for hosing down the floor. This side needs to have a backflow preventer on it. Repeat Violation

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime: - ice,chute on beverage machine in dining room. General Manager cleaned during in inspection. Corrected On-Site

Intermediate - Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink: - hand-wash sink in men's restroom no hot water. General Manager checked and it had been turned off underneath. General Manager turned on. Corrected On-Site

High Priority - Warewashing sanitizing solution exceeding the maximum concentration allowed. - three compartment sink quatrenary amonium tested to 400+ppm. Manager added water to dilute it. Retested to 200ppm. Corrected On-Site

Basic - Exterior door has a gap at the threshold that opens to the outside. - back door into establishment has a gap. Manager already has a work order in to have a sweep put across the bottom of the new door. Corrective Action Taken
