Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Upon inspection observed cell phone on prep counter in prep area of kitchen. Operator removed cell phone during inspection. Corrected On-Site

Basic - Floor tiles missing and/or in disrepair and/or in disrepair. Multiple areas of kitchen Cove molding damaged / missing in multiple areas and mop sink.

Basic - Floor area(s) covered with standing water. Back hallway leading to back door. Repeat Violation

Basic - Hole in or other damage to wall. Multiple areas with wall tile missing in kitchen.

Basic - Ice buildup in walk-in freezer.
