Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Upon inspection observed ice buildup inside walk-in freezer. Repeat Violation

Basic - Employee with no hair restraint while engaging in food preparation. Upon inspection observed both manager and drive thru employee not wearing hair restraints while handling food.

Basic - Equipment and utensils not properly air-dried - wet nesting. Upon inspection observed moisture between stacked plastic pans on drying racks.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Upon inspection observed ice buildup inside walk-in cooler.

High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. 2 small flying insects flying around kitchen and dining room. Did not observe flying insects landing on food or food contact area.

Basic - No handwashing sign provided at a hand sink used by food employees. No hand wash sign in men's bathroom. Printed copy for operator, operator hung sign in bathroom. Corrected On-Site

Basic - Wet mop not stored in a manner to allow the mop to dry. In mop sink area, mops sitting on mop sink, not properly drying. Operator placed mops on hooks to properly dry during inspection. Corrected On-Site

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink: - hand-wash sink by ice machine does not have paper towels. Manager had employee fill during inspection. Corrected On-Site

Basic - Bathroom facility not clean: - men's bathroom has standing water on 3/4 of the floor: in the stall around the toilet, around both urinals, and under the hand-wash sink. Floor drain does not appear to be overflowing. Manager had employee clean up during inspection. Corrected On-Site

Basic - Employee with no hair restraint while engaging in food preparation: - a couple of employees not wearing hats and/or hairnets.

High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.

Basic - Food stored on floor. Upon inspection observed multiple bags of ice stored on floor left side of walk in freezer.

Basic - Nonfood-contact surface soiled with dust. Walk in cooler fan covers.
