Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Buildup on ice sheild in ice machine.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime: - flat surfaces around some of the nozzles at the drive thru soda machine. General Manager had employee clean. Corrected On-Site

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance: - there are approximately 38 ceiling tiles in the dining room area that are stained and have a mold-like substance on them; - the ceiling tiles and vents in the kitchen are dirty with dust and some of the vents have a mold-like substance on them.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Walk in freezer and reach in freezer next to fryers in cook line area of kitchen. Repeat Violation

High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Upon inspection observed 2 small flying insects in kitchen. Did not observe landing on food/food contact surfaces.

Basic - Bowl or other container with no handle used to dispense food. Upon inspection observed small bowl resting in sauerkraut on sandwich prep line. Operator removed bowl during inspection. Corrected On-Site

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Upon inspection observed ceiling tiles with what appears to be water damage in dining area.

Basic - Equipment and utensils not properly air-dried - wet nesting. Upon inspection observed stack of pans with moisture between on top dry storage rack right side of 3 compartment sink. Operator removed pans and rewashed during inspection. Corrected On-Site

Basic - Equipment in poor repair. Gasket on small prep cooler left side of sandwich prep station in kitchen.

Basic - Ice buildup in walk-in freezer. Upon inspection observed ice forming on condenser and a mound formed on floor in walk in freezer.
