High Priority - Food preparation sink has soil/old food residue. Observed pieces of raw chicken splash on the inside food prep sink, must be cleaned more often during the day shift.

High Priority - Presence of insects, rodents, or other pests. Observed three flying insects in the prep area near two microwave ovens. Notify supervisor on duty next unannounced inspection.

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed mold like growth around the soda nozzles on the soda machine.(drive thru window) Observed liquid spilled on the side panel of the soda machine.

Basic - Floor soiled/has accumulation of debris. Observed old food stuck around pipe on the floor underneath prep sink. Floors inside walk in cooler has old food dried on the floor tiles.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed can opener has old food residue on the blade.
