High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee entered kitchen, put on gloves and began preparing food for customers without washing hands. Operator discussed with employee who then washed hands. Corrected On-Site Warning

High Priority - Rodent activity present as evidenced by rodent droppings found. Approximately 4 rodent droppings on floor in hallway leading to public bathroom. Approximately 2 rodent droppings on floor in storage closet in back of kitchen. 1 rodent dropping in kitchen on floor behind white chest freezer.
