High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed containers of raw chicken stored over containers of raw beef in the 2 door refrigerator in the back preparation area. The operator moved the chicken beneath the raw beef. Corrected On-Site

High Priority - Roach activity present as evidenced by live roaches found. Observed 6 live cockroaches in the up tube tray of the dish washing machine, 3 inside the pump chamber on top of the dish washing machine. Admin Complaint

High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed a line cook rinse off his gloves at the food sink instead of removing the soiled gloves and using new gloves. Discussed with the employee and operator. Employee removed the gloves and donned new gloves. Corrected On-Site

Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed the automated paper towels dispenser not activated when attempting to get paper towels. Operator indicates it isn't operable. Requested a stack of paper towels for the shelf by the hand washing sink. A stack of paper hand drying towels was provided Corrected On-Site

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed cracks in the ceiling of the back preparation room.

Basic - Dead roaches on premises. Observed 6 small dead cockroaches inside the pump motor chamber on top of the dish washing machine. Admin Complaint

Basic - Floors not maintained smooth and durable. Observed the floor panels lifting and separating in the walk-in refrigerator and freezer.

Basic - In-use knife/knives stored in cracks between pieces of equipment. Observed a large knife stored in the gap between the steam table and the adjacent counter. The operator removed the knife to the dish washing area for washing rinsing and sanitizing. Corrective Action Taken

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed food and grease buildup on the hand washing sink in the cooking area, the wire shelf under the 2 burner unit, the exteriors of the large single burner pot cookers,

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed a half moon of brown buildup in the ice scoop. Scoop taken to dish washing for cleaning.

Intermediate - Handwash sink not accessible for employee use at all times. 2 carts are parked in front of the dishwashing area hand washing sink. The carts were removed to make hand washing sink accessible Corrected On-Site

Observed a personal employee beverage on the shelf above the preparation table where tamales are being prepared. Another employee parley was found in the far back kitchen reach in refrigerator.
