Intermediate - Handwash sink not accessible for employee use at all times. Observed a 1/3 pan that had a tube of beef thawing in water in the sink. Manager removed the beef from the sink.

Basic - Cutting board has cut marks and is no longer cleanable. Observed the white cutting board with excessive grooves on the board.

High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed a splitter at the mopsink on which is attached a submersible hose that lacks a backflow preventer where that hose attaches.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed dust buildup on the ceiling vent above the pizza oven

Basic - Exterior door has a gap at the threshold that opens to the outside. Observed a large gap at the base of the back exterior door where at least 1/4 inch of light can be seen below the door sweep.
