Upon inspection, observed approximately 20 small flying insects inside of salad spinning machine on prep table in dishwashing area of establishment. No food items observed in machine. Manager quickly closed lid and removed machine from establishment to remove the insects. No obvious signs of small flying insects observed in other parts of establishment during inspection. Admin Complaint - From follow-up inspection 2025-10-21: Upon callback, salad spinning machine not observed at establishment. Manager states machine was removed from establishment. Employee states chicken has been out of cooler for approximately 30 minutes. Chicken was moved back into cooler during inspection. Also observed bean sprouts on front counter 64f. Employee states bean sprouts have been on the counter for approximately 2 hours and 45 minutes. Employee moved bean sprouts into cooler during inspection. Corrective Action Taken

Upon inspection, observed multiple containers in walk-in cooler containing food prepared at establishment with no date marking. Employee stated food has been in cooler for over 24 hours.

None of the items were labeled/segregated from food to be served to the public.
