High Priority - Operating with an expired Division of Hotels and Restaurants license. License renewed during inspection. Corrected On-Site

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Upon inspection observed inside door of ice machine with mold like substance present. Operator cleaned ice machine during inspection. Corrected On-Site

Basic - Floor tiles missing and/or in disrepair. Several in kitchen area.

Basic - Hole in or other damage to door. Upon inspection observed back kitchen door leading into back hallway in disrepair.

Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Upon inspection observed small metal measuring pitcher with handle resting in large container of sugar. Operator corrected storage during inspection.

High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Upon inspection observed 2 small flying insects by cashier area service window. No small flying insects observed landing on food/food contact surfaces.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Can opener with build up of food debris on prep table in kitchen next to upright coolers.

Basic - Ceiling tile missing. Back ok kitchen by fire sprinkler.

Basic - Cutting board has cut marks and is no longer cleanable. On top of small prep cooler right side of service window.

Basic - Employee with no hair restraint while engaging in food preparation.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Upon inspection observed small reach in cooler gasket next to upright coolers in back of kitchen with debris present.

Basic - Old labels stuck to food containers after cleaning. Upon inspection observed clean stacked plastic containers on drying rack in kitchen with old labels present.

Basic - Wall soiled with accumulated food debris. Next to hand washing sink by drive through service window.
