Basic - Single-service articles improperly stored. Observed to go boxes stored on the floor. The operator properly stored the to go boxes. Corrected On-Site

High Priority - Toxic substance/chemical improperly stored. Observed a bottle of chemical stored with single-service portion cups. The operator properly stored the chemical, Corrected On-Site

Basic - Floor not cleaned when the least amount of food is exposed.

Basic - Food stored on floor. Observed plastic water bottles stored on the floor. The operator properly stored the water bottles. Corrected On-Site

Basic - Garbage can soiled with an accumulation of food debris.

Basic - Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle.

Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed no chlorine test kit for the three compartment sink and sanitizer bucket.
