High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed rice and beans on steam table at 124F, operator stated items had been there approximately 30 minutes. Operator turned up the heat on steam table to bring temperatures back up. High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed no vaccum breaker on threaded hose Bibb at mop sink. Repeat Violation

Basic - Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed the 3 compartment sink not set up in order. Educated operator on correct order.
