Intermediate - Handwash sink used for purposes other than handwashing. Observed grease buildup in handwashing sinks next to grill.

Basic - Floor not cleaned when the least amount of food is exposed. Observed debris underneath equipment, racks, and cove molding along floors.

Basic - Food storage container/container lid soiled, cracked or broken. Observed lid on sugar with food debris build up.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up in walk in freezer around fan and condenser. Repeat Violation

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed on reach in cooler drawers, on line, build up of food debris and black mold like substance. Repeat Violation

Basic - Old labels stuck to food containers after cleaning. Observed old labels on food containers on dry shelf and drying shelf above three compartment sink.

Basic - Wet mop not stored in a manner to allow the mop to dry. Observed wet mop sitting in mop sink, mop head down.

Basic - Food stored on floor. Observed in walk in freezer and cooler food stored on floor. Per employee, truck came either yesterday or this morning. No definitive answer to question when asked.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets on reach in coolers soiled with food debris.

Marked exit/path to marked exit blocked. For reporting purposes only. Observed back exit door blocked with empty and full boxes.

Basic - Buildup of food debris/soil residue on equipment door handles. Observed on backside of make station handle with food debris buildup. Observed operator cleaning handle. Corrected On-Site

High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee changing gloves with out washing hand first

High Priority - Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee standing with hands on waste with gloves on. No changing of gloves, no handwashing.

Basic - Employee eating in a food preparation or other restricted area. Observed employee chewing gum while working on line.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed around the nozzles of the soda machine, the flat part with mold like substance on soda machine, also in ice chute located on soda machine. Drink station located in lobby.

Intermediate - Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Paper towel dispenser by flat top grill will not dispense paper towels. Observed Operator resetting dispenser and its operational now. Corrected On-Site

Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed one clear pan not inverted on drying shelf above triple sink. Observed employee correctly inverting pan for proper storage. Corrected On-Site

Basic - Fifty (50) foot-candles of light not provided at a food working surface or where safety is a factor. Observed two light bulbs out above grill station causing inadequate lighting.

Basic - Garbage on the ground and/or pad around dumpster. Observed garbage around dumpster.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice build up on ceiling of walk in freezer

Basic - Open dumpster lid. Observed dumpster lids open.

High Priority - Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. Observed prep table by hand washing sink not properly cleaned or sanitized after an employee was sitting on top of it working on the lap top. Observed employee cleaning and sanitizing said prep table. Corrected On-Site

Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee keys, handbag, above prep area and speaker on prep table during inspection. Observed General Manager removing speaker and personal items from prep table and shelf above. Corrected On-Site

Basic - Unpackaged food not protected from environmental sources of contamination during preparation. Observed employee sitting on prep table, next to hand washing sink, working on computer. Observed employee getting down immediately.

Basic - Ceiling tile missing. Observed ceiling tile missing by where you place order in lobby. Spoke with GM, and has repair company scheduled.

Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed brown water stains on ceiling tile above order station. Spoke with GM, and has repair company scheduled. Corrective Action Taken

Basic - Covered waste receptacle not provided in women's bathroom.
