High Priority - Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Observed an employee crack raw shell eggs with bare hands and then handle a spatula without washing. Operator had them was their hands, the spatula, and put on gloves. High Priority - Live, small flying insects found. Observed approximately 7 live small flying insects on the truck. They did not land on any food surfaces. Operator is currently attempting to combat the problem with sticky traps, air curtains, and treating the area outside. Warning

Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed a line cook with multiple bracelets in their wrists while preparing sandwiches. Operator had them remove their bracelets. Warning

High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed approximately 15 small flying insects in the warewashing section landing on to-go containers. Operator willingly disposed of the to-go containers and washed all landing spots. High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw ground sausage stored over ready to eat mushrooms in the reach in cooler. Operator moved the mushrooms. Corrected On-Site Warning

Basic - Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. Observed no visible number on the outside of the truck. Warning

High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw eggs stored over commuted sausage. Operator switched the positions. Corrected On-Site Warning

Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Observed no vomit clean up plan. Handed operator a plan. Corrected On-Site Warning
