High Priority - Operating with an expired Division of Hotels and Restaurants license. Observed their license expired on 12/1/25. Operator was on call with the accounting department during the inspection and there was an error with their initial payment and were only allowed to pay via check to renew, at this time they wrote the check and just have to mail it. High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed in the cold water on the line: Cooked onions (70F - Cold Holding); Cooked mushrooms (70F - Cold Holding) Operator stated they were out for approximately 30 minutes and placed them in the reach in cooler.
