Facility was under a Boil Water Advisory between 6/28/23 morning to 6/30/23 morning at 10am. The following items were prepared on 6/29/23 during the BWA without a Temporary Emergency Operation Plan in place: 5 metal containers of cheese dip, 4 plastic containers of cheese dip, 1 metal container of soft tortilla shells and 1 plastic container of chipotle sauce. These items were voluntarily discarded due to being unsafe and adulterated to COS. Food shall be safe, unadulterated and honestly presented.

improperly date marking prepared TCS (time/temp. control safety) foods. Containers of prepped meat and veggies were date marked with the day after being prepped. PIC relabeled all containers with correct prepped and discard date to COS. Marking the date or day of preparation, with a procedure to discard the food on or before the last date or day by which the food must be consumed on the premises, sold, or discarded.

Raw chicken in walk in freezer was stored in grocery bags and shrimp cocktail sauce in preptop cooler was stored in single use juice container. Utensils and food contact surfaces of equipment shall be made of materials that are safe, durable, corrosion resistant, and non-absorbent, and be able to withstand repeated ware washing and shall be finished to have a smooth easily cleanable surface.

Metal reach in freezer for drink glasses, under prep tables, shelves in walk in cooler and walk in freezer have a buildup of food residue and debris. Non-food-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Clean these non-food-contact surfaces and any other ones that have buildup of residue or debris.

Floor is kitchen is pitted and catching food residue/water. Replace and/or repair flooring where it is creating a pooling or catching food residue/water. All physical facilities shall be maintained in good repair. Walk in freezer floor is damaged.
