6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed multiple time/temperature control for safety (TCS) foods held in large make table over 41 F. CA: All TCS food must be held at 41 F or below for cold holding. COS: Person in charge (PIC) discarded all out of temperature foods and will only use the unit for non-TCS foods items until repaired.
18 - insects, rodents, and animals not present

Back door has broken gaskets and light gaps on side and bottom. REPEAT

Single use cup used as scoop in bulk seasonings.

Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Make table unable to cold hold at 41 F or below; Not in good repair. Ice build-up on door and fan in walk in freezer.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
