Observed ice machine not cleaned at a frequency necessary to preclude accumulation of residues. CA: Notified PIC. PIC will have front food service ice bins cleaned. Discussed cleaning frequency of ice bins.

Observed no paper towels available at hand sinks. Employees are drying hands with rags. CA: Notified PIC that paper towels must be available at all hand sinks to properly dry hands. Reusable rags can not be used to dry hands. PIC will purchase paper towels for the facility.

Observed chocolate milk, 2% milk, and diced ham inside of reach-in coolers not date marked after being opened. CA: Informed PIC that all TCS food items being held for 24 hours or more must be date marked for seven days. PIC placed dates on food items.

Observed multiple wet wiping cloths sitting out on counters. CA: Informed PIC that all wet wiping cloths must be placed in sanitizer solution in between uses.
