Observed lamp that does not work and food debris and dirt in the dresser drawer in room 103, dirty microwave in room 104, baseboards in need of cleaning in room 201 and food splatters in microwave in room. CA: All furnishings in fixtures must be maintained clean and in good repair. Corrections will be made prior to renting rooms to new guests

Observed mattress protector with stains and holes and dirt/debris on the bedding in room 103, bed sheets with holes and lady bugs in room 104, hair on sheets in room 105, stain on pillow cases in room 201 and stained mattress in room 205. CA: All bedding will be treated/cleaned/replaced before rooms are rented to new guests

Observed the hot water turned off at the bathroom sink, stains on bathroom towels, leaky bathtub faucet with tub finish peeling, sink and toilet in need of cleaning in room 105. Toilet paper roll holder fixed to the wall is broken and sink faucet is leaking in room 201. Bathtub has used bar of soap and toilet in need of cleaning in room 205. CA: Rooms must be updated and cleaned before they are rented out to new guests

Observed used toiletry items in room 205. CA: Items will be discarded and replaced with individually wrapped toiletry items

Weather stripping around exterior door is peeling off in room 105. CA: The PIC will repair/replace weather stripping

Observed AC vent with brown tar-like build up in room 205. CA: Vent will be cleaned prior to renting to new guests

Observed annual gas-fired equipment inspection from July 24, and no new inspection completed or producible. CA: Gas-fired equipment inspections are to be completed annually and equipment maintained in good repair

Observed in room microwaves in rooms 103, 105, 201, and 202 in need of cleaning (accumulations on top portion of interior of microwaves). CA: All furnishings in rooms must be clean and in good repair

Observed pieces of plastic and crumbs, and stain on bedding in room 201. CA: All bedding must be clean and in good repair in rooms

Observed no slip mat or slip resistant surface in room 202. CA: All tubs/showers must have a slip resistant surface in floor to prevent slips

Observed live insects in between clean bedsheets and blankets in room 221. CA: Discussed with PIC. Licensed pest control will be called out to service the room and the room will not be available to rent until re-checked by the health department

Observed towel rack in disrepair and not fastened to the wall in room 221. CA: PIC will repair before the room is available to be rented

Observed the door that connects the adjacent room in disrepair; the deadbolt is missing and paper towels have been used to fill the hole in the door; the door frame is broken and a long piece of metal or wood is protruding from it. CA: Discussed with PIC. PIC will repair the lock and doorframe before the room is available to be rented
