Observed opened bottles of shampoo in rooms 101, 105 and 107. CA: All toiletries in guest rooms shall be individually wrapped. PIC removed opened shampoo
23 - documentation and records available for inspection
Regulation: 511-6-2-.13(2) - insect and rodent control-records
Observed PIC unable to produce documentation of monthly pest control services from the last 6 months. The only records available are from December 2024 and February 2025. CA: The PIC will email the records and moving forward they must be available for health department review during the inspection
25 - floors, walls, ceilings, windows, doors clean, and in good repair; appropriate for location and operation
Regulation: 511-6-2-.14(1) - floors, walls, and ceilings
Observed bathroom finishes in disrepair in room 105 (wall behind toilet, tile/caulking beside the bathtub, paint lifting by mirror). CA: Repairs will be made by the next routine inspection
32 - separate, structurally sound, clean, and in good repair: dryers vented and adequate lighting
Regulation: 511-6-2-.17(1) - laundry facilities
Observed significant dust and lint build up on the vent behind the dryers. CA: The vent will be serviced before the next routine inspection and maintained more frequently to prevent build up
33 - housekeeping carts and storage areas maintained; clean/sanitized items protected from contamination
Regulation: 511-6-2-.17(2) - housekeeping carts
Observed all working spray bottles of toxic items on the house keeping cart with no label and another bottle in the laundry room with no label. CA: PIC will have all bottles labeled by the next routine inspection
