Observed opened bottles of shampoo in rooms 101, 105 and 107. CA: All toiletries in guest rooms shall be individually wrapped. PIC removed opened shampoo

Observed PIC unable to produce documentation of monthly pest control services from the last 6 months. The only records available are from December 2024 and February 2025. CA: The PIC will email the records and moving forward they must be available for health department review during the inspection

Observed bathroom finishes in disrepair in room 105 (wall behind toilet, tile/caulking beside the bathtub, paint lifting by mirror). CA: Repairs will be made by the next routine inspection

Observed significant dust and lint build up on the vent behind the dryers. CA: The vent will be serviced before the next routine inspection and maintained more frequently to prevent build up

Observed all working spray bottles of toxic items on the house keeping cart with no label and another bottle in the laundry room with no label. CA: PIC will have all bottles labeled by the next routine inspection

Observed loose faucet in the hand sink in room 101. CA: Fixtures shall be maintained in good repair

Observed water damage on bathroom ceiling . CA: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed personal drinks stored on housekeeping carts. CA: Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from contamination

Observed microwave in room 107 in need of cleaning or replacing. Observed bag of limes in fridge unit in room 119. CA: Discussed with PIC, all in room facilities should be maintained clean

Observed mattress cover in room 107 with stains. CA: Discussed with PIC, if linens cannot be treated to remove stains, they need to be replaced

Observed pest control records for December and February. CA: Discussed with PIC, pest control needs to occur monthly by a licensed pest control company with summaries

Observed debris in fridge in room 113, and a canned drink and water bottle in room 133. Discussed with PIC the need to check and clean out fridges and microwaves between guests, and to clean room appliances between guests

Observed cigarettes butts, food debris, dryer sheets, and dryer lint in parking lot and in walk ways around facility. Discussed with PIC to increase frequency of parking lot and walk way cleanings to maintain areas free of litter
