EHS requested to view the facility's gas-fired equipment inspection. The PIC provided an inspection report performed on 02/01/2024 by Watson Plumbing. The PIC indicated that a company has been contacted to perform a new inspection, and are scheduled to perform the inspection on Thursday, January 15.

511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency.

CA: EHS advised the PIC that gas-fired equipment is to be inspected annually by a qualified licensed contractor or local fire authority, if available, and that an inspection is to be acquired as soon as possible

EHS observed the following in each room:

• Room 103 - Hairs on the mattress pad, sheets, and mattress cover; stains on the mattress pad, pillow, and mattress cover.
• Room 121 - Stains on mattress cover and mattress pad; hairs on the mattress cover.
• Room 227 - An apparent burn hole in the comforter; stains on the mattress cover and pillow.
• Room 217 - Tears in the mattress cover.
• Room 302 - Stains on the mattress pad and sheets.
• Room 304 - Stains on the mattress cover and mattress pad

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.

CA: EHS advised the PIC that all linen items are to be clean and in good condition, without stains and/or holes. The PIC is to ensure that housekeeping is inspecting the linen items—removing stained or damaged items from use. Linen with stains should either be re-cleaned or discarded and replaced. Linen with holes/tears should either be repaired or discarded and replaced

EHS requested for gas fire report. PIC provided a report that was past due for a inspection. The last fire inspection was February 1, 2024.

CA-PIC needs to contact a certified plumbing company to service their property and make sure that the inspection is being completed annually.

EHS observed the following in each room: Room 211-stained mattress pad Room 219- brown stain on mattress pad Room-217- hair on mattress pad Room 307-stained mattress pad Room 319-yellow stain on mattress pad Room 321-yellow stain on mattress pad

CA-PIC needs to make sure that housekeeping is changing all bedding items in each room. No bedding items should stained. If stains do not come out they need to properly discard the item and replace them.

EHS observed the following in each room:

Room 211-microwave had stains inside of the unit on the door. Room 222-microwave had stains inside of the unit on the door. Room 211-stained sofa (black spot) Room 117-microwave had stains inside of the unit on the door.

CA-PIC needs to make sure that employees are routinely cleaning the furniture in each room. Any items that are damaged or stained must be replaced

EHS observed the following in each room:

Room 225-stained mattress pad. Room 314-stained comforter and hair on mattress sheets. Room 327-stained mattress sheets.

CA-PIC needs to make sure that housekeeping is properly cleaning each room and changing sheets. Bedding items must be free from any stains or debris. If items are damaged then they must be properly discarded and replaced with new ones.

EHS observed stains and damage to outside material of curtains in rooms 205, 206, 320, including paint stains and dark ink-like stains to inner plastic liners.

EHS observed damage and water leaking around feet of ice machines of floors 1 and 3.

All furniture and must be maintained in good repair, clean, and free of vermin.

EHS advised PIC to change out indelibly stained curtains in affected rooms. EHS advised PIC to repair or replace ice machines to prevent water pooling and possible future water damage

EHS observed lack of gas-fired equipment inspection report.

PIC informed EHS that gas-fired inspection report was not available. EHS advised PIC to contact plumbing service to have equipment inspected and obtain resulting report

EHS observed stains on inside of plastic liner on curtains in rooms 205, 304, 318, 319.

Violation Code Reference 511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.

EHS instructed PIC to clean or replace stained liners or curtains entirely

EHS observed stains on pillowcases in rooms 202, 203, 205, 318, 319.

EHS observed stains on sheets in room 202.
