EHS observed the following:

• 2 live cockroaches in the bathroom and kitchenette areas of room 314
• Several cockroaches in rooms 324, 411, and 405

PIC indicated that the 4th floor was recently sprayed by their pest control provided which they believe the observed dead cockroaches be be attributed to.

511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program.

CA: EHS advised the PIC that pest control services for room 314 should be increased in-order to eliminate the current presence of cockroaches and prevent future harborage. Rooms within which dead cockroaches were observed should be cleaned; although the observed rooms were recently sprayed, the facility should ensure no dead cockroaches are present in the rooms an appropriate time after the rooms are serviced

EHS observed the following: Dirty air filters-Rooms 309,328,424 Rusting appliances (microwave)-Rooms 309,402 Rusting appliances (Fridge)-Room 309, 328 No and/or deteriorating caulking around kitchen and bathroom sink-Room 304

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.

CA: Advised PIC to ensure air filters are cleaned regularly. Advised PIC to repair and/or replace appliances observed to be rusting. Advised PIC to have new caulk applied around areas observed during inspection. Discussed the need of continue maintenance with PIC

EHS observed the following: Debris on mattress pads-Rooms 433, 428, 404, 331 Hairs on bed coverings-Rooms 433, 402, 331 Stains on sheets-Room 328, 429 Holes in bed coverings- 328

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.

CA: Advised PIC to have linen containing debris cleaned and/or changed. Informed PIC that items with holes or those in which stains remain should be discarded and replaced

EHS observed air filters in all rooms except 415 (see comments for room numebrs) needing to be cleaned. EHS observed undergarments and odor in drawers in room 415

EHS observed debris on mattress pads in the following rooms : 224,201, 415. Washable mattress pads or covers shall be used on all mattresses. EHS observed insufficiently hot water in rooms 314 (85), 311 (80), 310 (92), and 413 (86). Temperatures in fahrenheit in parens.

511-6-2-.07(2) - Hot Water Supply Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F.

EHS instructed PIC to increase temperature of boilers or have a professional inspect hot water system

EHS observed dumpster side-doors open.

511-6-2-.12(2) - Garbage and Refuse Containers All outside refuse or garbage storage containers shall be constructed of durable metal or other approved types of materials, which do not leak and do not absorb liquids and shall be provided with tight-fitting lids or covers and shall be kept covered when stored. Each container shall be located within 100 feet of dwelling units or trailer spaces or in a location approved by the Health Authority and shall be cleaned at such frequency as to prevent a nuisance or odor.

EHS instructed PIC to ensure that side-doors on dumpster are shut

EHS observed refuse in and around dumpster corral including pallet, empty bottles, miscellaneous trash. PIC informed EHS that dumpster facility is under heavy use.

511-6-2-.12(3) - Garbage and Refuse Container Cleaning Adequate cleaning facilities shall be provided and each garbage or recycling storage room, enclosure, or container shall be thoroughly cleaned after the emptying or removal of refuse or garbage. Areas surrounding these rooms, enclosures, and containers shall be kept clean and orderly. Liquid waste resulting from the cleaning of containers shall be disposed of as sewage.
