  • 2 live cockroaches in the bathroom and kitchenette areas of room 314
  • Several cockroaches in rooms 324, 411, and 405

PIC indicated that the 4th floor was recently sprayed by their pest control provided which they believe the observed dead cockroaches be be attributed to.

511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program.

CA: EHS advised the PIC that pest control services for room 314 should be increased in-order to eliminate the current presence of cockroaches and prevent future harborage. Rooms within which dead cockroaches were observed should be cleaned; although the observed rooms were recently sprayed, the facility should ensure no dead cockroaches are present in the rooms an appropriate time after the rooms are serviced
