EHS observed prep top unit and reach in cooler, underneath, to not ne maintaining proper temps; EHS observed temps to be at 57F at PT and 51 for RIC.

3. Ambient air temperature, water pressure, and water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use.

CA: EHS advised the PIC that the unit needs to be serviced; PIC notified their supervisor.

EHS observed no food service permit displayed. Violation: Post the permit in a location in the food service establishment that is conspicuous to consumers. CA: post food service permit in public view

EHS observed a few dead and alive insects throughout the facility.

Corrective action- PIC needs to make sure that all insects and pest are properly removed from the facility.

Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.

EHS observed two reach in coolers that leaking onto the flow. One reach in cooler had a puddle of water that leaked over to the 3 compartment sink.

Corrective action- PIC needs to contact maintenance and address both leaks under the reach in coolers.
