Upon beginning of inspection, EHS observed dishes stored clean with food debris on food contact surface of serving plates.

PIC instructed employee to remove any unclean dishes and re-wash. This was COS. PIC also stated that before opening of facility, dishes will be checked to ensure of cleanliness.

EHS observed heavy ice buildup on condenser in WIF. PIC informed EHS that they were aware of issue. PIC asked EHS if they are able to put a catch pan under. EHS informed PIC that we don't want to use any equipment we use for food.

EHS observed heavily stained and multiple scratches on 3 cutting boards at prep top coolers in main kitchen. Cutting Surfaces. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they cannot be resurfaced. Informed PIC to either have cutting boards resurfaced or replaced. This must be corrected by 10/31/24

Observed 2+ employees working without a beardguard. (i) Hair Restraints.
Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.
This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles. EHS informed PIC of the need for all hair longer than 1/2 an inch to be restrained.

Observed multiple containers of food stored uncovered on the food prep line. (iv) Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. EHS informed PIC that all food must be stored covered;Male employee placed lids on all containers.

Observed male dish washer working with 2 bracelets on his wrist and one male cook with an apple watch on his wrist. (g) Jewelry. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. EHS informed PIC that no jewelry is allowed on the hand/wrist except for a plain wedding band.

(a) Equipment and Utensils, Air-Drying Required. After cleaning and sanitizing, equipment and utensils:
Shall be air-dried or used after adequate draining before contact with food; and
May not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry.
EHS informed PIC; PIC stated he observed the issue prior.
