28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered
Regulation: 511-6-2-.14(6) - bed, bedding, and linens
EHS observed the following:
- Room 107 - Stained mattress pad and sheets, hair on sheets
- Room 109 - Stained mattress pad and sheets
- Room 111 - Stained mattress pad
- Room 113 - Stained and ripped sheet
- Room 141 - Stained sheets
511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.
CA: Advised PIC that observed linen items are to removed and cleaned, and replaced with fresh linen. Torn linen items are to be repaired to original condition or discarded and replaced. If linen remains stained after multiple washes they are to be discarded and replaced

EHS observed that the DPH TA permit was not displayed for public view. The facility was unable to locate the permit during the inspection.
511-6-2-.04(1)a - Permit Display Any person operating a tourist accommodation shall obtain and display a valid DPH tourist accommodation permit issued by the Health Authority. (b) To qualify for a permit, an applicant shall:
- Be an owner of the tourist accommodation;
- Allow access to the tourist accommodation;
- Provide all information as required on the application and pay all applicable local and state fees referenced in DPH Rule 511-6-2-.22 at the time the application is submitted and;
- Score 100% on the permitting inspection for newly constructed facilities.
CA: Advised PIC that the facility is to display TA permit for public at all times. EHS will provide new copy of permit for facility's use
19 - anti-slip tubs/showers, slip strips, mats or appliqué products provided and in good condition
Regulation: 511-6-2-.08(10) - anti-slip surface
EHS observed that the anti-slip mats at the bottom of the shower/tub were visibly dirty/stained and worn, and in need of replacement in all rooms inspected but 141.
511-6-2-.08(10) - Anti-Slip Surface Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair.
CA: Advised PIC that anti-slip mats should be discarded and replaced when they become visibly worn, stained, or dirty, and when cleaning methods no longer restore their original condition
