EHS observed the following:

• Room 107 - Stained mattress pad and sheets, hair on sheets
• Room 109 - Stained mattress pad and sheets
• Room 111 - Stained mattress pad
• Room 113 - Stained and ripped sheet
• Room 141 - Stained sheets

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.

CA: Advised PIC that observed linen items are to removed and cleaned, and replaced with fresh linen. Torn linen items are to be repaired to original condition or discarded and replaced. If linen remains stained after multiple washes they are to be discarded and replaced

EHS observed that the DPH TA permit was not displayed for public view. The facility was unable to locate the permit during the inspection.

511-6-2-.04(1)a - Permit Display Any person operating a tourist accommodation shall obtain and display a valid DPH tourist accommodation permit issued by the Health Authority. (b) To qualify for a permit, an applicant shall:

1. Be an owner of the tourist accommodation;
2. Allow access to the tourist accommodation;
3. Provide all information as required on the application and pay all applicable local and state fees referenced in DPH Rule 511-6-2-.22 at the time the application is submitted and;
4. Score 100% on the permitting inspection for newly constructed facilities.

CA: Advised PIC that the facility is to display TA permit for public at all times. EHS will provide new copy of permit for facility's use

EHS observed that the anti-slip mats at the bottom of the shower/tub were visibly dirty/stained and worn, and in need of replacement in all rooms inspected but 141.

511-6-2-.08(10) - Anti-Slip Surface Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair.

CA: Advised PIC that anti-slip mats should be discarded and replaced when they become visibly worn, stained, or dirty, and when cleaning methods no longer restore their original condition

Observed that the light switch for the bathroom in room 201 did not turn light on after several attempts. Light eventually came on after several attempts. Toilet rooms, lavatories and bathing facilities shall be provided with soap, artificial light, and hot and cold water under pressure. Have repaired by 7/11/2025

Toilet rooms, lavatories and bathing facilities shall be provided with soap, artificial light, and hot and cold water under pressure. Have repaired by 7/11/2025

EHS observed temperatures exceeding 120F in all rooms inspected.

511-6-2-.07(2) - Water Supply and Sanitizing - Hot water in all guest rooms shall not exceed 120F.

EHS advised PIC to turn down boilers in order to bring facility into compliance

EHS observed damaged and stained iron boards in rooms 205 and 113. EHS observed a stained lampshade in room 217. EHS observed damaged or otherwise unhygenic AC unit filters in rooms 119, 217, 213.

All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation...must be maintained in good repair, clean, and free of vermin.

EHS advised PIC to replace or clean affected furniture

EHS observed damaged and stained sheets in rooms 135, 133, 213. EHS observed damaged and stained pillowcases in rooms 135, 133, 226.

511-6-2-.14(6) - Bed, Bedding, and Linens

Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin.

EHS advised PIC to replace end-of-life sheets and pillow cases that cannot be effectively cleaned, increase cleaning frequency of both, and better screen sheets that come from smoking rooms for cigarette burns

EHS observed a cracked ink in room 205, and tarnished/damaged drain fixtures in all rooms observed.

511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance

Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair.

EHS advised PIC to repair or replace fixtures listed

Violation: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. Inspector note: EHS observed rusty appliances and furniture in all rooms. EHS observed cushioned chairs with debris under cushion in all rooms. EHS observed Corrective action: EHS instructed PIC to change out appliances with non rusted appliances. PIC in the process of renovating and removing stated items. PIC will have employees vacuum the cushioned chairs

Violation: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Inspector note: EHS observed varying degrees of linen with hair (rm 111, 113, 115, 129, 128) stained spots (rm 111, 113, 125, 129, 128) and holes in linen (rm 128, 130). Corrective action: EHS instructed PIC to have linen clean, free of vermin, and in good repair

Violation: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists. Inspector note: EHS observed enlarged shower heads with varying degree of mold/rust (rm 111, 113, 124). EHS observed all rooms with older drains with varying degree of tarnish or rust. Corrective action: PIC will have drains and shower heads repaired/replaced

Violation: Toilet, lavatory and bathing facilities shall be mechanically ventilated. Inspector note: EHS observed all rooms without lavatory ventilation. Corrective action: EHS instructed PIC to have lavatory ventilation
