EHS requested for the pest control records. PIC showed EHS a text message stating that they had sprayed the facility but PIC could not provide any receipt or document that includes who serviced them, what date, and what chemicals were used.

Corrective action- PIC needs to contact the certified pest company to service the facility. Also keep a physical documentation of each time the facility is serviced.

  3. If present, playground equipment and associated areas shall be maintained in a clean and sanitary condition. In addition, a cleaning and sanitizing plan along with cleaning and sanitizing records for playground equipment and associated areas shall be maintained within the food service establishment for review upon request by the Health Authority.
