EHS observed the following violations in the listed rooms:

• Room 210 - lights hairs on the sheets
• Room 211 - lights hairs on the sheets
• Room 223 - red stain on the mattress pad; black steak on the sheets
• Room 224 - light brown spot stains on the sheets; burn hole on the mattress pad

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.

CA: EHS advised the PIC that the noted linen items are to be removed, cleaned, replaced with fresh linen. Linen items from which stains can't be removed should be discarded and replaced. Linen with holes are either be repaired, if possible, or discarded and replaced

EHS observed a dark, mold-like substance on the interior ice door/flap of the ice machines on the 1st and 2nd floor, which require cleaning.

511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice.

CA: EHS advised the PIC that the ice interior portion of the must be cleaned and sanitized; recommended that the facility increases the cleaning frequency

EHS observed room 111 hot water at 126F. Violation: Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F. CA: adjust water heaters to ensure temperature does not exceed 120F in any guest rooms

EHS observed lack of gas-fired equipment inspection report on premises. Violation: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency CA: ensure licensed contractor or local fire authority completes inspection annually and form is available upon request

EHS observed dust accumulation on air conditioners vents or filters in all rooms observed. Violation: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. CA: clean filters and vents at frequency to prevent dust accumulation

EHS observed couches being kept next to pool building that needed to be discarded. Violation: Grounds, including spaces beneath buildings and trailers, shall be kept clean and free of litter. CA: remove couches from property if needing to be discarded

EHS observed facility without permit displayed. Violation: Any person operating a tourist accommodation shall obtain and display a valid DPH tourist accommodation permit issued by the Health Authority CA: post permit in public view. If copy of permit is needed contact Health Department and a copy will be sent

EHS observed facility without inspection report posted. Violation: The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away. CA: print and post inspection report from 07/28/25

EHS observed ice machines on 1st and 2nd floor in need of cleaning. Violation: Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. CA: clean and sanitize ice machine according to manufacture's procedures

EHS observed facility using Washitaw Pest Group with Marcus Swayne Trust who is not licensed per Dept of Ag license search. Violation: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program. CA: Replace pest control provider with a licensed pest control provider

EHS observed lack of gas-fired inspection report on premises. Violation: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. EHS observed AC Vents with dust accumulation in all rooms observed. Violation: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. CA: Clean filters at a frequency to prevent dust build up

EHS observed stains or hair on all arm chairs and/or couches in rooms observed. Violation: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. CA: Clean or replace furniture that can no longer be cleaned

EHS observed bedding in need of cleaning in all rooms observed: 121: Skrewball bottle under mattress; debris under mattress 201: debris under bed/on mattress 207: Stained sheets Violation: eds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. CA: ensure that bedding and mattress are kept clean and if become uncleanable due to residue or stains to replace

EHS observed facility not displaying Tourist Accommodation Permit. Violation: Any person operating a tourist accommodation shall obtain and display a valid DPH tourist accommodation permit issued by the Health Authority. CA: Print and Post Permit sent via email to facility

EHS observed stained carpets in every room inspected. EHS observed stains on upholstery of provided chairs in rooms 104, 106. EHS observed stains behind mirror closet door in room 112. EHS observed stained desk surface in room 105.

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation...must be maintained in good repair, clean, and free of vermin.

EHS advised PIC to replace or thoroughly clean carpets and upholstery, as well as have housekeeping address the rear of closet doors
