EHS observed pink mold like substance on baffle in ice machine when wiped with alcohol pad. Violation: food contact surfaces of equipment and utensils shall be clean to sight and touch. CA: Drain, clean, and sanitize ice machine.

Violation: 1. Single-service and single-use articles may not be reused. Inspector note: EHS observed facility reusing boxes as drip pan and reusing single serve plate to hold bacon. Corrective action: PIC will get mats to prevent slipping and drip pans to collect spills. PIC will have bacon placed in a food-grade container.
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Violation:1. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Inspector note: EHS observed ice accumulation in the walk-in freezer and two reach-in freezers. Corrective action: EHS instructed PIC to have units cleaned as frequently as to prevent such.

Violation: 1. Dressing rooms or dressing areas shall be designated and used if employees routinely change their clothes in the establishment. 2. Lockers or other suitable facilities shall be provided and used for the orderly storage of employees' clothing and other possessions. Inspector note: EHS observed employee personal items in the reach-in cooler (turkey bacon), walk-in freezer (gatorade), and walk-in cooler (glucerna). Corrective action: EHS instructed PIC to have designated areas in locations of employee personal item usage to prevent potential contamination of consumer items.
