EHS requested pest control records from the PIC. PIC stated that they changed companies and they do not have the pest control inspection report at the property.

CA- PIC needs to make sure when they are being serviced by pest control that they have a copy of the pest control inspection reports and keep it on file and up to date.

511-6-2-.13(2) - Insect and Rodent Control-Records A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises, except for the occasional use of consumer insect sprays on small spots in accordance with the product label. The record may be made by the permit holder or provided by the applicator, and shall list the following information: (a) amount and concentration of product used; (b) name of product used; (c) date and location of application; (d) application method; (e) pest targeted; and (f) name of applicator
