Observed the air gap for the 3-C sink blocked by a subway cup. Explained to the PIC that the air gap must be accessible at all times. Per PIC, will contact maintainance who placed cup in the gap to have removed. Cup should be removed.

Observed small pieces of old labels remaining on the outside of pans stored as clean above the 3-C sink. During the wash process, all labels and associated residue shall be removed. PIC is to remove and rewash.

Observed heavy black splash and and sticking staining on the outside of the prep sink in the back kitchen area. All non-food contact surfaces of equipment shall be cleaned at a frequency to prevent accumulations. PIC is to clean.

Observed the need to replace the ceiling tiles above the ovens/proofer along the back wall of the facility. Ceiling tiles are stained with brown/black smudges. Physical facilities shall be cleaned at a frequency to prevent accumulations. Per PIC, facility has attempted to clean the tiles multiple times. Suggested that if cleaning is not working tiles should be replaced.

Observed the coving nearest the 3-C sink and mop sink no longer attached to the wall. All physical facilities shall be maintained in good repair. PIC will submit maintenance request for repair.

Upon entering facility, EHS observed employee prepping sandwiches with bracelet on right wrist. (g) Jewelry. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food.
EHS informed employee to remove bracelet, wash hands and change gloves. Employee removed bracelets and washed hands. This was COS.

Violation: 1. Except as specified in paragraphs 2 and 3 of this subsection, food shall be protected from contamination by storing the food: (i) In a clean, dry location; (ii) Where it is not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 cm) above the floor. Inspector note: EHS observed (2) boxes of chips directly on the floor. Corrective action: PIC informed EHS they were the only employee in for the day. PIC would finish duties during slow time of the day.
