1-2B - certified food protection manager
Regulation: 511-6-1.03(3)(b) - certification requirements/exemptions (pf)
EHS observed facility without proof of CFSM. Per PIC class will need to be scheduled by their manager. Violation: At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall be a certified food safety manager who has shown proficiency of required information through passing a test that is part of an accredited program that conforms to the national standards for organizations that certify individuals. Certified Food Safety Managers must be designated to one food service establishment only and maintain and renew certification in accordance with the requirements of the examination taken. CA: Obtain CFSM by 03/27/26.
2-2A - management knowledge, responsibilities, reporting

EHS observed facility did not have a verifiable means that employees were informed of conditions they must report. Violation: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. CA: Have all employees sign employee health reporting form and keep on site to be made available upon request.
2-2E - response procedures for vomiting & diarrheal events
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
EHS observed facility unable to provide materials with corresponding instructions for addressing fecal/vomitus events. Violation: A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter. CA: Obtain kit with procedures for fecal/vomitus events.

EHS observed the facility without most recent inspection report displayed. Violation: The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. CA: Print and post inspection report from today 01/27/26.

EHS observed facility without food service permit displayed. Violation: Post the permit in a location in the food service establishment that is conspicuous to consumers. CA: Display permit if you no longer have a copy of permit contact office for a copy of the permit.
17B - garbage/refuse properly disposed; facilities maintained
Regulation: 511-6-1.06(5)(n) - covering receptacles (c)
EHS observed both side doors open on outside dumpster. Violation: When not actively taking trash out side doors shall be kept closed. CA: PIC will close doors.
