PIC unable to provide gas-fired equipment inspection report for view at time of inspection. PIC indicated that the facility has had the annual inspection performed but inspection report was misplaced by staff.

511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

EHS observed the following:

• Room 118 - Stains on sheets
• Room 109 - Stains on sheets and pillow cover; stains on box spring cover; hairs on sheets and mattress pad
• Room 216 - Stains on pillow cover and sheets; hairs on sheets
• Room 210 - Stains on pillow cover, sheets, and mattress pad
• Room 319 - Hairs on sheets and mattress pad
• Room 326 - Stains on sheets, mattress pad, and mattress cover; hair on sheets

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.

CA: Advised PIC that observed linen items with stains and/or hairs are to be cleaned and changed. If stains are unable to be removed from linen they should be discarded and replaced. Emphasized the importance of housekeeping checking linens during cleaning and bed-making to minimize instances of hair being left behind

Observed that the current Gas Inspection on file was dated 6/4/24 therefore it is no longer considered as being current. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, Have inspection completed by 7/72025

Inspector note: EHS observed facility without gas-fired inspection report on file.

Corrective action: PIC will have plumber come out and redo inspection to provide report.

EHS observed the following in each room:

Room 206-Microwave and fridge needs to be clean. Room 302-Yellow stain in microwave located on the door. Room 303-Microwave needs to be cleaned.

Corrective Action- PIC needs to make sure that housekeeping is cleaning all the furniture that is located in all the rooms.

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

EHS observed the following in each room:

Room 107-Stains on mattress sheet, mattress pads, and hair on mattress pad. Room 206-Holes on mattress sheets. Room 207-pink stain on mattress sheets. Room 303-Hair on blanket and mattress pads.

Corrective action- PIC needs to ensure that while housekeeping is cleaning each room that they are changing the bed items with clean items. If any mattress items are damaged they need to be properly discarded and replace with new ones.

EHS observed that the facility did not have up to date hotel inspection report located at the facility. The inspection report they had post by the front desk was from 2021.

Corrective action- PIC needs to make sure that they have a a copy of the most up to date hotel inspection report located at the facility.

511-6-2-.05(7) - Inspection Posting Any tourist accommodation inspection report addendum(s), completed by the Health Authority to document observations, violations, and corrective actions resulting from an inspection need not be displayed, but must be made available by the tourist accommodation operator to the public upon request

EHS requested for up to date pest control records. Facility was unable to provide documentation.

Corrective action- PIC needs to ensure that they have up to date pest control records located at the facility.

511-6-2-.13(2) - Insect and Rodent Control-Records A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises, except for the occasional use of consumer insect sprays on small spots in accordance with the product label. The record may be made by the permit holder or provided by the applicator, and shall list the following information: (a) amount and concentration of product used; (b) name of product used; (c) date and location of application; (d) application method; (e) pest targeted; and (f) name of applicator

Inspector note: EHS observed rooms 118, 116, 112, 109, 107 and 110 with varying degree of oil stains and/or hair on bedding. Corrective action: PIC immediately replaced bedding in question
