8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
PIC unable to provide gas-fired equipment inspection report for view at time of inspection. PIC indicated that the facility has had the annual inspection performed but inspection report was misplaced by staff.
511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency
CA: Advised PIC that gas-fired inspection report must be available for view at time of inspection
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

EHS observed the following:
- Room 118 - Stains on sheets
- Room 109 - Stains on sheets and pillow cover; stains on box spring cover; hairs on sheets and mattress pad
- Room 216 - Stains on pillow cover and sheets; hairs on sheets
- Room 210 - Stains on pillow cover, sheets, and mattress pad
- Room 319 - Hairs on sheets and mattress pad
- Room 326 - Stains on sheets, mattress pad, and mattress cover; hair on sheets
511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.
CA: Advised PIC that observed linen items with stains and/or hairs are to be cleaned and changed. If stains are unable to be removed from linen they should be discarded and replaced. Emphasized the importance of housekeeping checking linens during cleaning and bed-making to minimize instances of hair being left behind
