6-1B - proper hot holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; hot holding (p)
Observed pizza hot holding in cabinet with internal temperature of 121F. Cabinet was not turned up highenough and pizzas were not at 135F when put in the cabinet. CA reheat pizza to 165F or discard.
17A - toilet facilities: properly constructed, supplied, cleaned
Regulation: 511-6-1.07(5)(i) - closing toilet room doors (c)
Oserved bathroom door propped open. Either bathroom door or locker room door that goes to the kitchen must be kept closed at all times, except for cleaning.

Regulation: 511-6-1.07(2)(h) - walls & ceilings, studs, joists, & rafters, not exposed to moisture (c)
Observed ceiling tile in locker room and bathroom with water and/or mold damage
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(3)(f) - lighting intensity, adequate in food prep, storage & service areas (c)
Observed inoperable light fixtures in walk in freezer. At least one of the bulbs is shattered inside the shield.
