At time of inspection, the door of the mini-refrigerator in Room 116 was difficult to open/close (it appears the door may be warped/bent). PIC made note to have maintenance assess the refrigerator for repair/replacement

At time of inspection, observed stains on the mattress protector in Room 331. COS: PIC removed all of the bed linens and instructed housekeeper to replace all of the bed linens

At time of inspection, facility has not had annual gas-fired inspection performed. PIC states she has had difficulty getting a company to come out. PIC states she will contact another company and have them come out ASAP.

4/4/2025: Upon checking back with the GM, she informed me that Dekle Plumbing had come out and performed the gas-fired inspections. She emailed the completed forms to me and I have added them to the facility's folder. This violation has been corrected

At time of inspection, observed a very loose handle on the shower door in Room 106. COS: PIC had maintenance come and tighten the handle

At time of inspection, observed smooth bottom of tub in Room 116 (it appears the texture has been smoothed over time and with cleaning). Apply applique or replace texture, to prevent slipping in the tub

At time of inspection, PIC could not provide any inspection record showing that all gas-fired equipment has been inspected at least annually. CA: PIC is contacting company to begin inspecting all gas-fired equipment at least annually. DPH gas-fired equipment inspection form provided to PIC

At time of inspection, observed a loose tile in the bathroom in Room 201. COS: Head Housekeeper notified maintenance and maintenance person immediately re-attached the tile to the floor
