Observed multiple pots and utensils with food debris on the interior and/or exterior. COS: PIC removed items from service and had them washed/rinsed/sanitized again.

Damaged concrete in front of kettles only partially patched. CA: The floor should be repaired like the concrete was repaired in front of the tilt skillets on the opposite side of the cook line - complete and entirely in one large patch to fix the area instead of small patches here and there.

Exhaust hood ventilation system not working above the cook line. When the switch is flipped, you can hear some component of it try to turn on but no suction occurs to remove contaminants. CA: Must be serviced and fixed by 23 FEB 2026.

Observed gaps between wall and floor in outside WIF and damaged concrete at the drain grate in the kitchen at the cooking equipment.

CA: Charlton EH instructed CFSM to have the concrete sealed to prevent any spills/leaks from seeping into the exposed concrete. CFSM had maintenance start caulking the outside WIF. CFSM stated that the concrete floor in the kitchen was scheduled to be sealed in two (2) days and they would have that company repair the other damaged sections of concrete.

Observed dead roaches on premises.

CA: Charlton EH instructed PIC to have FSE cleaned and suggested to keep outside doors into dining hall closed.

Observed feces on ground in kitchen.

CA: Charlton EH ordered the entire kitchen to be immediately cleaned and sanitized. The only reason why the FSE was not immediately shut down is because there is no food being served nor is there any food on site. PIC stated that contractors were in the kitchen the day before working on equipment and it looked like one of them scraped off their shoe on what they thought was a floor drain.

Multiuse food contact surfaces are not smooth, or are not free of breaks, open seams, cracks, chips, inclusions, pits, and similar imperfections, or are not free of sharp internal angles, corners, and crevices.

CA: Charlton EH instructed PIC that before reopening, new utensils and pots/pans must be obtained.

Observed gaps at ceiling in dry storage and dust and dirt build-up.

CA: Charlton EH instructed PIC to have gaps sealed and to clean kitchen before they are allowed to reopen.

Bathroom vents not working. Vents are also obstructed due to dust or paint.

CA: Charlton EH instructed PIC to have vents repaired and cleaned.

Observed water leakage from ceiling above high temp. dish washer. CA: Maintenance to repair leak.

Debris on floors/shelves in dry storage and dust build-up in exhaust chute in electrical room. CA: PIC stated all equipment, shelves, and utensils to be cleaned and sanitized before use. Door to electrical room to remain secure during operation.

MULTIPLE DEAD ROACHES DISCOVERED IN KITCHEN AREAS: WALK-IN-COOLER #1, CHEMICAL STORAGE, MAIN KITCHEN, ELECTRICAL ROOM, AND DRY STORAGE.

CA: FACILITY IS NOT ALLOWED TO SERVE FOOD UNTIL SUCH TIME THAT THE PEST ISSUE IS ADDRESSED. TOLD PIC TO HAVE PEST CONTROL COMPANY ASSESS THE SITUATION AND DEVISE A PLAN TO ERADICATE THE ROACH POPULATION MOST LIKELY LIVING IN THE WALLS AND HARBORING IN THE UNUSED EQUIPMENT.
