4-1A - food separated and protected
Regulation: 511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii) - packaged & unpackaged food separation, packaging, and segregation (p, c)
Observed raw, shelled eggs and raw bacon stored above RTE American slices, tomatoes, and onions. Also observed raw ground beef stored above RTE lasagna.
RCA: Ensure to store RTE food items above raw animal foods. Then arrange raw animal foods in the order of minimum cook temperatures.
1-2B - certified food protection manager
Regulation: 511-6-1-.03(3)(d) - certified food safety manager responsibility
Observed expired CFSM certificate posted in the facility. PIC was unable to provide evidence of any other CFSM employed at the facility.
RCA: A CFSM certificate which has expired, been revoked or suspended shall not be posted in the food service establishment. Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM) to ensure food safety is being managed within the food service establishment during all hours of operation. A CFSM certificate which has expired, been revoked or suspended shall not be posted in the food service establishment.
6-2 - proper date marking and disposition

Observed sliced tomatoes, sauteed onions, chicken salad, and lasagna stored in the cooler for longer than 24 hours without a date mark.
RCA: Ensure all TCS and RTE food items are date marked if stored for longer than 24 hours.
