Observed all rooms to have a water temperature of over 120F.

RCA: Told PIC to try to get water temperature turned down. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F

Observed stains on mattress pad and pillow in some rooms.

RCA: Told PIC that they need to ensure that all linens are clean. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

PIC was unable to produce documentation for annual gas fired equipment inspection. Stated that facility does not routinely get equipment inspected. RCA: Contact a qualified licensed contractor to inspect all gas fired equipment in facility. Inspection is required annually. Template emailed to PIC

Observed staining on mattresses in rooms, 101,129,221, and 224.

Told PIC to ensure that clean or place mattress and sheets when there is heavy staining

Observed multiple doors in stairwell have a gap between bottom of door and floor. Observed ants and roaches around first floor door leading outside. RCA: Seal all doors and exit ways tightly to ensure no pests are able to enter

Observed staining on pillows (all rooms), mattress pad (room 101, 205, 308 and 322), comforter (room 103), and mattress (room 101 and 308). RCA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed baseboard behind the toilet to be broken and falling off of the wall. RCA: Baseboard shall be repaired. Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair

Observed mold -like growth on bathroom ceiling above front area of tub of room 308. RCA: Bathroom shall be treated for the mold-like growth on ceiling. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture

Observed black and pink slime-like debris on the baffle of 1st floor ice machine. RCA: In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations

Observed a live spider in the tub of room 231 and a small bug in the ice bin in room 307.

RCA:511-6-2-.13(1) - Insect and Rodent Control Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents

Observed staining on the chairs in all rooms inspected. Observed a condom wrapper under the mattress cover in room 231. Observed old food debris in the microwave of room 307. Observed a Q-tip and part of a Newport cigarette box under the mattresses in room 325.

RCA: 511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

Observed staining on the mattress covers in room 123. Observed a stain on the comforter in room 202. Observed staining on the pillows in room 307. Observed burn stains on the sheets in room 325.

RCA: 511-6-2-.14(1) - Floors, Walls, and Ceilings Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed a mold-like buildup on the baffle of the 1st floor ice machine.

RCA: PIC took ice machine out of service to be cleaned and sanitized. 511-6-2-.07(8) - Ice Supply If self-service ice is provided, then it shall be from an approved water supply. In all new tourist accommodations, and in existing tourist accommodations when machines are replaced, only automatic dispensing ice machines will be allowed. In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean. Glassware shall not be used to scoop ice
