Observed staining on pillow in room 320. Observed staining on two pillows in room 222. Observed staining on mattress cover in room 127.

CA: PIC had staff replace soiled linens and pillows with clean ones. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean

Observed scratches and worn slip-resistant surface in basin of tub in room 320. Observed tub surface in rooms 220 and 229 to have worn slip-resistant basins.

RCA: Tubs shall be re-surfaced to prevent any potential hazards. Tubs with scratches shall be re-surfaced to be easily cleanable and sanitized.

CA: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair at all times

Observed the sitting chair to have a build up of debris in the cracks in room 317. Observed the reclining chair to have debris under the cushion in rooms 334 and 431. RCA: All furniture must be maintained and kept clean

Observed a build up of debris on the baffle of the self-service ice machine. RCA: The use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations

Observed brown staining on mattress covers in rooms 121 and 223.

RCA: PIC will ensure bedding is removed prior to use and will discard if stains cannot be replaced.

Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed staining and other debris on the mattresses and linens in rooms 121, 221, 214, 213, 210, 306, 309, 332. CA: PIC will ensure housekeepers treat the staining, and if it cannot be treated it will be replaced
