1-2B - certified food protection manager
Regulation: 511-6-1.03(3)(c) - certification documentation (pf)
Observed the Food Safety Certification document to be expired and posted in clear view.
COS CA: PIC shall obtain a valid and non-expired certification for food safety and post it in clear view. PIC has ninety days of the expiration of the CFSM certificate, the CFSM shall enroll in an approved food safety training course, pass an approved exam and obtain a new certificate.
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed sugar in dry-storage to be rolled close with opportunity for pests and contamination to enter. Observed oreo crumbles stored in cabinet adjacent to drain to be uncovered and unprotected from potential pests or contamination.
COS CA: PIC covered sugar bag with protective covering before storing in dry-storage area. PIC covered oreo crumbles in cabinet as to protect it from pests and contamination.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed ice machine to have black build-up on baffle.
RCA: PIC shall burn all ice in ice machine, clean and sanitize. PIC shall make one batch of ice and burn. The second batch of ice shall be used. All food contact surfaces shall be clean to the sight and touch.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(6)(n) - manual and mechanical warewashing equipment, chemical sanitization-temperature, ph, concentration, hardness (p,pf)
Observed a sanitizer concentration of 0ppm.
COS CA: PIC replaced sanitizer with a bleach solution that shall be tested to ensure the concentration is at correct concentration.
17A - toilet facilities: properly constructed, supplied, cleaned
Regulation: 511-6-1.07(5)(h) - cleaning plumbing fixtures- handsinks, urinals & toilets (c)
Observed restroom facilities to have biological build-up around base of toilet, and surrounding floor.
RCA: Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary to keep them clean and maintained.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed personal items including medicine, cell phones and cups to be in the food-prep area.
COS CA: PIC moved all personal and staff items to a designation location away from food-prep area.
