Observed container of pooled raw shell eggs stored beside cooked shrimp and above noodles in bottom of prep top cooler. COS: PIC had employee place eggs on bottom shelf away from cooked foods.
2-2D - adequate handwashing facilities supplied & accessible

Observed handwashing sink near ware washing area to not have soap available. RCA: Ensure soap is readily available at hand washing sinks at all times.
4-2A - food stored covered

Observed several items throughout kitchen to be uncovered while not in active use (pork and eggrolls in reach-in cooler, salt and msg on prep table, veggies and pork in bottom of prep-top cooler). COS: PIC had employees cover items not in-use.
