Observed TCS whipped butter packets out of temperature control in the bagging area with a TPHC sticker from 11/2 (2 days old). Temperature of butter 79F. Stated butter likely came out at 5 or 6am that morning. COS: PIC discarded butter and started a new container from cooler with correct 4-hour time sticker. Rule: The food shall be marked accurately or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control.

Rule: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Inspector's Note: Observed build up of debris on floor of walk in cooler and in cabinets next to fryer. Notified PIC. PIC instructed employees to sweet up area. PIC will implement cleaning schedule to address noted areas for increased cleaning.

Observed the mops to be stored in the mop buckets and not in use.
COS/RCA: PIC removed mops and hung to dry. Drying Mops. After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.
