2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(c) - handwashing sinks, installation (pf, c)
Observed the hot water handle to be stripped and unable to operate at the front hand sink.
RCA: PIC stated that maintenance it scheduled to come out on upcoming Tuesday (5/19/26) to repair. The other hand sink in the back of the kitchen is operational for the time being. Rule: A handwashing sink shall be equipped to provide tempered water at a temperature of at least 85ºF (29.4 ºC) through a mixing valve or combination faucet.

Observed a spatula and food residue in the basin of the front hand sink.
CA: PIC removed the spatula from the hand sink. PIC shall have hand sink cleaned and sanitized. Rule: A handwashing facility may not be used for purposes other than handwashing.
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed pizza stored on racks without overhead protection. Observed breadsticks stored on top of prep counter without overhead protection. Observed container of olives stored in the reach-in cooler without a lid.
CA: PIC placed lids and protective coverings over all items. Rule: Food shall be protected from contamination by storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed "clean" cambros stored on dish rack while still greasy to the touch with residue remaining on the surfaces.
RCA: PIC shall ensure all cambros are being cleaned thoroughly to remove all soils and residues. Rule: Equipment food-contact surfaces and utensils shall be clean to sight and touch.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed food employees working without hair restraints or beard nets.
RCA: PIC shall order hair/beard nets or other hair restraints for employees to use. Rule: Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(g) - jewelry (c)
Observed food employees with bracelets and watches.
Rule: Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee food stored on top of covered single use/service plastic cups. Observed employee drink (soda bottle) stored on the shelf with single use/service plastic cups. Observed employee phone on the prep counter. Observed a speaker on the prep counter. Speaker was relocated to the shelf with single use/service articles.
CA: Employee food and drink were moved to area away from food service items. Employee phone was removed from prep space. PIC advised to designate a place to store employee items to prevent contamination to food and food service items. Rule: Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination.
