Observed crumbs and debris in the drawers in rooms 305, 319, 304, and 332. Also observed stains on the curtains in room 303 and debris in the cabinets in room 332.

Observed stains in the tub in rooms 305, 319, and 304, while build up on the faucet is observed in room 322.

Observed marks, cracks, and stains on the walls of room 304 and stains on the ceiling of room 121. Also observed a hole on the door in room 304.

Observed black build up on the baffle of the ice machine. RCA: Ensure the ice machine baffles are clean at all times

Observed dead insects on the floor of room 103 and 123. Observed dead insects in the dresser drawers of room 111. CA: PIC provided pest control records from Yates-Astro where the last service occurred on 12/11/2024. Advised PIC to ensure housekeeping vacuums rooms properly

Observed gas fired equipment and PIC was unable to provide the gas fired equipment inspection at the time of inspection. Provided PIC with gas fired equipment inspection form

Observed stains on pillow in room 103, and stains and cigarette burn hole on pillow in room 105. Observed dark red droplet like stains on the mattress in room 105. Observed rip all the way along one side of the mattress seam in room 111. Observed stain on the mattress pad in room 117. RCA: Advised PIC to ensure all bedding are clean and in good condition at all times

Observed inspection report from 4/29/21 posted out of public view. RCA: The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away

Observed shower rods to be broken in rooms 103, 121, and 123. Observed shower floor to be cracked in room 105. RCA: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair

Observed the doors of rooms 111 and 105 to not be sealed to the outside. RCA: Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors

Observed build up of debris on the floors of rooms 103, 105, 111, 121, and 123. Observed a white crystalline substance in two corners of room 105. Observed a build up of a purple waxy substance on the floor of room 111. Observed tile on the bottom of the wall in room 123 to no longer be attached to the wall. Observed rubber covering on the bottom of the wall to be missing and peeling away in room 105. Observed a splatter like stain on the door and wall of room 105. RCA: Advised PIC to ensure all walls, floors, ceilings, and doors are clean and in good condition at all times

Observed black build up on the baffle of the ice machine. RCA: Advised PIC to follow manufacturers recommended cleaning schedule for the ice machine. Advised to adopt a more frequent cleaning schedule for the ice machine if necessary

Observed hot water temperature above 120 F in rooms 109, 107, 105, 103, and 101. Advised PIC to ensure water temperatures not to exceed 120 F at all times

Observed dead insects in the chest or nightstand drawers of rooms 117, 101, 103, and 107. Also observed live insect in 107. Advised PIC to maintain regular pest control methods to reduce the presence of pests

Observed holes and/or stains on the bed spreads, fitted sheets, top sheets, pillows, mattress covers, and mattresses in rooms 117, 101, 105, 107, and 109. In addition, a wiping cloth was made in the bed of room 117. Advised owner to ensure all bedding are clean and in good condition at all times

Observed blood on the door inside room 123; stains and/or paint removed on the walls/ceilings of rooms 109, 107, 105, 103, 101, and 117. Advised owner to ensure all walls, floors, ceilings, and doors are clean and in good condition at all times
