Observed dust and debris under the bed in rooms 4 and 5. Also observed nail polish on the walls and floors in room 12. RCA: Ensure all walls, floors, and ceilings are clean and in good repair at all times

Observed dresser in room 16 and 28 to have swollen walls due to moisture exposure. Observed several dressers missing knobs on dressers. Observed cracked refrigerator and missing microwave plate in room 1. RCA: 511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

Observed staining, debris, holes and burn marks on pillows, comforters, sheets, mattress pad, mattress cover, mattress, box spring and bed skirts in all rooms inspected. RCA: 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed build-up around sink and drain in room 4. Observed broken toilet lid and slow draining toilet in room 16. Observed debris in bathroom and peeling ceiling paint in room 28. RCA: 511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists

RCA: 511-6-2-.14(1) - Floors, Walls, and Ceilings Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored

Gas-fired equipment inspection report is not observed in the facility during inspection. Advised PIC to have gas-fired inspection report available on site at all times

Observed missing hardware on furniture in room 28; observed food in the microwave in room 10, unclean microwave and fridge in rooms 11, 4, and 2

Observed slow drain in the face bowl in room 1. Missing shower ring in room 28; broken molding and toilet seat fixture not in good repair in room 11. Stains in bathtub in all inspected rooms. Advised PIC to ensure all restrooms are clean and in good condition at all times
