6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed multiple items in the walk-in cooler holding above 41 degrees F.
CA: Hot dogs, chicken wings, liquid eggs, mayonnaise, milk, creamers etc, were discarded on site. RCA: The walk-in shall not be used until repaired. Outside freezers shall be used as the main unit for holding until the inside walk-in unit is repaired. PIC/CFSM shall notify the Health Authority once the unit is repaired to clear for use. Rule: Time/temperature control for safety food shall be maintained at 41°F (5°C) or below
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(c) - handwashing sinks, installation (pf, c)
Observed no hot water available at the back hand sink next to the ice machine.
RCA: PIC shall have hot water restored to the hand sink to allow for proper hand washing for employee's. Rule: A handwashing sink shall be equipped to provide tempered water at a temperature of at least 85ºF (29.4 ºC) through a mixing valve or combination faucet
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed brownish and pink build-up on the baffle of the ice machine.
RCA: PIC shall ensure the ice machine is emptied, washed, rinsed and sanitized to prevent build-up. Rule: Equipment food-contact surfaces and utensils shall be clean to sight and touch.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed build-up of residue on the inside of the soda nozzles for the soda machine in front food service area.
RCA: PIC shall ensure nozzles are being removed and cleaned daily to prevent build-up. Rule: Equipment food-contact surfaces and utensils shall be clean to sight and touch.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.02(1)(d) - displaying of the inspection report (c)
Observed the inspection report from two inspections ago on display and not the most recent report.
CA: PIC shall post the most recent inspection report in the lobby and at the drive-thru window. Two copies of the current inspection report provided for posting in both areas.
Rule: 1. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. 2. Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information.
14C - single-use/single-service articles: properly stored, used
Regulation: 511-6-1.05(10)(e)1&3 - single-service/single-use items stored 6" off floor in clean, dry location (c)
Observed multiple boxes of single service items (cups, napkins) stored in the floor of the outside dry storage room.
Rule: Single-service and single-use articles shall be stored: (i) In a clean, dry location; (ii) Where they are not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 centimeters) above the floor.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed build-up of food debris inside the reach-in coolers and freezers. Observed pooling of water and food debris and Cvap holding unit. Observed build-up of food debris inside the warmer drawers.
Rule: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
15C - nonfood-contact surfaces clean

Observed build-up of food debris in the floors in the walk-in coolers.
RCA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed missing ceiling tiles in the front food service area.
Rule: All physical facilities shall be maintained in good repair.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(2)(c) - floor and wall junctures, coved, & enclosed or sealed, water-flushed floors provided with drains & graded to drain (c)
Observed broken floor tiles at the threshold between walk-in cooler and walk-in freezer. Observed missing floor tiles in front of the 3-compartmetn sink.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
