Observed staining on mattresses in rooms 110, 206, and 319. Observed stain on quilt in room 319. Observed stain on mattress pad in room 110.
RCA: PIC shall have soiled linens replaced and stains on mattresses treated. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin
15 - ice from approved source; properly handled, supplied and stored; equipment approved/clean

Observed ice machine on second floor to have slime-like and mold-like growth on baffle.
RCA: PIC shall have ice machine cleaned and sanitized per manufacturer's instruction. CA: Ice machines shall be maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations. Further, a copy of the manufacturer’s cleaning instructions shall be maintained onsite for review at the request of the Health Authority
