Observed hand towel in hand washing sink.

CA: PIC moved hand towel to designated area. Wiping cloths laundered in a food service establishment that does not have a mechanical clothes dryer shall be air-dried in a location and in a manner that prevents contamination of food, equipment, utensils, linens, and single-service and single-use articles and the wiping cloths.

Observed reach in cooler gaskets to have heavy build up and debris.

RCA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Observed multiple items throughout the walk-in cooler temping above 41F. See temperature log. COS: PIC discarded items prepped more than 4 hours ago and placed recently prepped items on ice to rapidly cool. PIC called maintenance person to service cooler.

Observed live roach on floor of ware washing area. RCA: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

1. Routinely inspecting incoming shipments of food and supplies;

2. Routinely inspecting the premises for evidence of pests;

3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and

4. Eliminating harborage conditions.

Observed air curtain in walk-in cooler to be missing flaps.

RCA: All physical facilities shall be maintained in good repair.

Observed 2 live roaches underneath freezer in back kitchen. Observed coving missing from wall and hole in wall right beside freezer that could be potential harborage area. Observed fruit flies around ware washing area.

RCA: (k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

(a) Good Repair. All physical facilities shall be maintained in good repair.

(b) Cleaning, Frequency and Restrictions.

1. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.

2. Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing.

Observed multiple floor tiles lifting up in grill area causing some residual food build up on the floor.

RCA: 511-6-1.07(2)(a) - Floor, Walls, & Ceilings, Cleanability (C) (2) Design, Construction, and Installation.

(a) Floors, Walls, and Ceilings, Cleanability. Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
